You are strongly encouraged to complete the required medical and dental history forms via our secure patient portal. Please bring pertinent dental records, X-rays, etc., to your appointment, especially if you seek a second opinion.

You will receive an appointment-reminder phone call or email 2-3 days prior to your scheduled appointment.
